Lorri,your print head is going bad but you may be able to correct some of the issues. I had similar test prints and found out that the wipers were pushing dried ink to the end of the head covering some of the nozzles. I had to manually remove that…

I have the same model.   Pour some more cleaner into the captops and see if they drain. Do test print and post a pic here - I'll tell you what to do with the head then. A bad captop will cause droplets and drop outs.
